VINEYARD HAVEN, MA — Hospice & Palliative Care of Martha’s Vineyard (HPCMV) and Navigator Homes of Martha’s Vineyard share a simple belief: that Island residents should be able to stay here, close to the people and places they love, through every stage of life. That shared belief is the foundation of their growing partnership.

Navigator Homes opened its doors in February, welcoming its first residents to a place many in this community have been working toward for years. Set on 26 acres in Edgartown, Navigator Homes offers long-term care, short-term rehabilitation, assisted living (certification pending with the Massachusetts Executive Office of Aging & Independence), and memory care across five home-like cottages, each with just 14 residents, private rooms, en-suite baths, family-style dining, and 24/7 skilled nursing. From the start, HPCMV has been on-site, providing hospice, palliative care, and grief support so that residents and their families have everything they need, right where they are.

This year, Navigator Homes stepped forward as a Presenting Sponsor of the 16th Annual Bud Lamoreaux Memorial Hospice Golf Classic, one of HPCMV’s signature fundraising events, joining co-Presenting Sponsor Scotts Miracle-Gro. The sponsorship was made possible through the generosity of a donor who cares deeply about both organizations. Proceeds support HPCMV’s care programs across the Island.

Polly Brown, a founding force behind Navigator Homes and a Board Emeritus member of HPCMV, has long believed these two organizations belong together. Her commitment to both is part of what makes this partnership feel so natural.

Melinda Loberg, another Navigator Homes Founder and Board Secretary, and former member of the HPCMV Board of Directors notes, “As a Board member of Navigator Homes of MV, I’m personally touched and grateful for the services provided by HPCMV.  Many of us Islanders share a profound appreciation for this Hospice organization and believe our partnership will strengthen the support that each of our organizations strives to provide to the individuals and families we serve.”

“Navigator Homes has become a cornerstone of our island community, and we are honored to walk alongside their residents and families during life’s most profound moments. The care we are able to provide, right where people live, is made possible through relationships like this one,” said Chantale Patterson, RN, BSN, OCN, CHPN, Executive Director of Hospice & Palliative Care of Martha’s Vineyard.

Last year, HPCMV cared for 92 patients and supported 107 adults and 40 youth through grief counseling and community programs. In September 2025, the organization was re-accredited by the Community Health Accreditation Partner (CHAP) with no deficiencies, a result that HPCMV’s team worked hard to earn.
